Transition

Perfect for: Children transitioning for the first time from home or clinic environments to the school system.

  • Individualized Education Plan (IEP) Preparedness: Help parents understand how to ensure their child’s needs are fully reflected in the Individualized Education Plan (IEP)
  • Transition Timeline: Provide a clear step-by-step roadmap for the transition.

In home Parent Consultation (90 min)

Meet parents or service providers to assess challenges/ opportunities, provide a summary of recommendations.

Develop a Detailed Action Plan

Prepare a detailed plan of action to implement the recommendations provided. This includes a step by step guide to implement the recommendations, materials (if needed) to implement the recommendations provided during the initial consultation.

In-home Session to Model Strategies (90 min)

Demonstrate the implementation of the prepared Action Plan.

Follow up in-home sessions for review, continuing training/ coaching (60 min)

Small Group Workshops (4-10 people, 90 min)

Topics including navigating IEP’s, building home-school collaboration, effective behavior strategies, etc

Preparation for Individualized Education Plan (IEP)/ Admission Review & Dismissal (ARD) Meeting

Review of documents – Full & Individual Evaluation (FIE), previous IEPs, data collected, outside therapy notes, data from all service providers

Advocacy and Support During Individualized Education Plan (IEP) meetings
